New Patients
When you call to make an appointment, please have your insurance card ready. We accept most major insurance and can verify your eligibility before you come to the office. Dr. Kavanagh also sees patients at the Long Island College Hospital Movement Disorders clinic, and some patients can be accommodated there.
The forms you will need can be downloaded and printed out. If you prefer, we will mail them. Please fill out the questionnaire before your visit, or arrive 20 minutes before your scheduled time. This will optimize your time with the doctor. We also encourage you to bring all your medications to your first visit, and to write out your questions before the visit.
Although our office is at street level, there is a small step at the entrance. Patients who are wheelchair-bound should discuss access with us before making an appointment so that we can be sure that you can enter and exit safely.
